Plan Overview
Indiana Masonic License Plates

Indiana State Bureau of Motor Vehicles issues Masonic Plates to applicants who present an approved eligibility authorization form with the other necessary documents and fees when buying their Indiana Plates.  BMV charges a fee which is collected when plates are purchased. ($12.00 at year 2000)

Two Masonic Grand Lodges are recognized by the Indiana Department of Motor Vehicles.  Each makes it's own eligibility requirements, procedures and fee structure. Plates issued are identical except for the alphabetic plate number prefix.   ie:

  • MM prefix for Indiana Grand Lodge F&AM
  • PH for Indiana Grand Lodge, Prince Hall.
Our Lodge is subordinate to Indiana Grand Lodge F&AM. All info about Masonic Plate Program found on this we-site applies only to that Grand Lodge.

Indiana Grand Lodge F&AM verifies the plate applicant as eligible for Masonic "Group Recognition" Plates by approving an "authorization form."  It collects a $10.00 fee which is used to support various Masonic Programs and cover administrative costs. (in 1998-2000, $6.00 of the $10 was returned to the applicant's Local Lodge.)

Who May Apply
For Indiana Masonic Plates

Indiana Grand Lodge F & AM will consider applications for authorization verification from persons falling under it's Masonic jurisdiction or in catagories mentioned below:

  • Good Standing Indiana Masonic Lodge members
  • Widows of members of Indiana Lodges.
  • Indiana Order of the Eastern Star members
  • Certain dependent residents of Masonic Households
  • Indiana resident sojourning Masons. 
Note that added info probably will be requested and more processing time needed to verify good standing of  membership in any other catagory than Indiana Masonic Lodge Members.

Sorry.  Indiana BMV does not offer a choice of  License Plate numbers under this program due to it's method of pre-positioning blocks of plate numbers at Local License Branch Offices.

  • Recreational Vehicles
  • Trucks (weight under 11,000 lbs.)
  • Sorry, Not Motorcycles (a plate size issue)


What Forms
Where do I get them

Two different forms are used by Indiana Grand Lodge to verify your authorization to receive a Masonic plates or  obtain a renwal sticker from the Indiana Bureau of Motor Vehicles. 

See sample forms on this web-site..  Obtain "Official" forms from the Indiana Grand Lodge Office, your Lodge Secretary, or from publications of Grand Lodge, such as the Indiana Freemason Newspaper.  Each year, Grand Lodge mails renewal forms to those who purchased plate the previous year.

The 2 forms are almost identical.

The New or Additional Plate form is used to order a new or another metal license plate and has a place for you to show the name of the local license branch to which you wishes the plate to be delivered.  Copies of it are often printed in the Indiana Freemason newspaper.

The Renewal form , is used to order  a renewal sticker for an existing plate.  So, there is a place on it to show the License Plate Number which is being renewed.  Indiana Grand Lodge regularly sends out blank renewal forms to plateholders well in advance of plate renewal dates.

  3. __ Show local license branch if new plate
  4. __ Show county of residence
  5. __ Is Name and Signature on the form the same as those on the vehicle registration? (including spouse, etc.)
  6. __ Check payable to Indiana Grand Lodge?
  7. __ Self addressed & stamped business sized envelope  enclosed?
