Home Assistance Program

The Home Assistance Program (HAP) offers help each day with the physical and social needs of elders and disabled adults living in designated housing units. 

HAP is an option that recognizes and preserves the independence of persons who wish to remain in their home, but who require assistance with daily activities to live safely on their own.

HAP services provided include:

  • Personal care -- bathing/dressing
  • Medication Monitoring
  • Walking/Transferring Assistance
  • Meal Preparation
  • Light housekeeping
  • Laundry and/or shopping
  • Access to transportation

All persons referred to the program are carefully screened by HAP's professional staff, who work closely with referral sources to assist with eligibility determination, client qualification for MassHealth, and other arrangements.

Criteria for Eligibility
Participants must:
...need
daily assistance with personal care
...be at least 22 years of age
...have a medical condition or a physical challenge
...must be eligible for MassHealth (Medicaid)
...must live in housing specifically designated or adapted for those who are elderly or disabled
