Formerly the Putnam County Health Services Clinic. A UNITED WAY AGENCY. A not-for-profit community agency funded by grants received from the Indiana State Department of Health for Nurse Managed Care, Primary Health Care for Children and WIC. Family Planning Services are funded by the Department of Health and Human Services, Public Health Service, Title X Family Planning Project and Indiana Family Social Service Administration, Title XX. The Clinic is a recipient of United Way monies and other local donations. In accordance with Federal law and U.S. Department of Agriculture, the Clinic does not discriminate on the basis of age, sex, race, color, creed, national origin, disability, religion, number of pregnancies, marital status, contraception preference, or source of payment for care.

Immunizations, Physical Examinations, Diagnosis and Treatment of minor illness, Dental Referrals, Family Planning Services, Screening for Lead Poisoning, WIC, Special Classes on Nutrition Education and Breastfeeding, Prenatal Care Coordination, and Social Work Services available for families.
